For the initial registration process, successful candidates are required to complete the following steps:
Notice of Admission (NOA): This document is essential for identification purposes.
Duly Accomplished Oath Form or Panunumpa ng Propesyonal: Candidates need to fill out this form as part of the registration process.
Two (2) Pieces of Passport-sized ID Photos: These photos should have a white background and include the complete name tag.
Two (2) Sets of Documentary Stamps: Required for processing the registration.
One (1) Piece of Short Brown Envelope: This is needed for organizing and submitting the required documents.
Verification of the rating for the Licensure Examination for Certified Plant Mechanics involves face-to-face oath-taking for newly registered professionals.
